I have general questions about the printing and delivery of the SPOT forms. Who do I contact?

Which course sections are never evaluated with SPOT forms?
SPOT forms are not printed for the following course sections:
Sections - 294, 491, 494, 498, 499, 599, 591 Course - GTA 500.
If you would like SPOT forms for one of these courses please check with your department chair and if acceptable the department chair can request Client Technology Services (962-2007)to print the forms.
I need extra SPOT forms for a particular class. Where do I get these forms and how do I fill them out?
SPOT forms are produced for each faculty member listed in Banner by course and section number. The number of SPOT forms produced for a class is consistent with the number of enrolled students. If for some reason you require additional forms:
- Pick up additional forms from Client Technology Services in Hoggard 234.
- Fill in the following information on the form, marking the corresponding bubbles.
- INST - The instructor number. This number is located on SeaNet ‘Search for Courses’ Tab (https://seanet.uncw.edu/PROD/swkfccl.p_sel_crse_search ) under ‘Instructor’. First determine how many instructors are listed. The first or primary (P) instructor is always INST 01, the second, INST 02. If this is the third, it will be INST 03 and so forth. Enter the appropriate INST number for this instructor.
- Course Code - This is the course call number. This number is located on SeaNet ‘Search for Courses’ Tab (https://seanet.uncw.edu/PROD/swkfccl.p_sel_crse_search ) under ‘CRN’.
- Semester - The semester to be evaluated, Fall, Spring, Summer I or Summer II.
- Year - the semester year.
- You may enter the course number and section as well as the instructor's name if you wish. This is not used in the actual evaluation but can be helpful in hand sorting or for visual reference during the evaluation.
If your class is finishing early, have your department chair or departmental secretary notify Client Technology Services (962-2007). They will print the forms which will then be available for pickup in Hoggard 234.
I have not received the SPOT forms for my department. Who do I contact?

If a class has two instructors identified on SeaNet and the department would like SPOT forms for only one instructor, what do I do?
The SPOT forms are printed for all courses except those that are requested by a department chair through the Faculty Senate SPOT coordinator to be excluded, or if the class section is one of several course sections that are always excluded. If an instructor's name appears on SeaNet, that instructor will have forms printed for that course. If you do not want an instructor evaluated, do not use the forms for that instructor.
