Name Changes
Bring a copy of your driver's license, social security card, divorce decree, voter registration card, passport, or other legal documentation verifying the name change along with a photo ID to the Registrar's Office and complete the Name Change Form.
Note: All name changes must be made in the Registrar's Office, located in James Hall.
