Name Changes
Current or Former, Temporary or Permanent Staff & Faculty need to bring the documents listed below to Human Resources (1612 Military Cutoff Road).
Graduate Student Employees need to bring the documents listed below to the Graduate School (2nd Floor, James Hall).
Undergraduate Student Employees need to bring the documents listed below to Career Services (Fisher University Union 2035).
All other non-employees need to bring the documents listed below to the Registrar’s Office (James Hall) and complete the Change of Name Form.

other legal documentation verifying the name change
