A. Procedure

Preparation of Stipend Payment Certification

The Stipend Payment Certification must be prepared according to the Instructions for Preparation of Forms. Certifications not properly completed will be returned to the department for correction.

Certifications must be attached to a Check Request (see Procedure 05.105) and forwarded to the Comptroller for payment.

Purpose

Use this form to support Check Requests covering stipend payments to individuals not employed by the State of North Carolina.

Where to Send Completed Forms

The original of the completed form, together with the Check Request, should be forwarded to Accounts Payable. Retain a copy for departmental records.
