Re-Registration - Officer Update Form - No Longer in Use!

Officer Update Form - This form is no longer a part of the re-registration process. INSTEAD, we have partnered with ITSD to implement an online management system for our student organizations. Student organizations will now be provided with a unique username and password (not related to any UNCW email ID's and/or passwords) and will then be responsible for logging on to the system and updating their organization's information by the re-registration deadline of Monday, September 28th, 2009.

Usernames/Passwords will be available by Wednesday, August 12th, 2009 and will be provided in person to a student officer with a UNCW student ID.

Once your organization enters its information into the system the first step in the re-registration process is complete, HOWEVER, it is the responsibility of each organization to make sure that organization information is kept current throughout the year. We ask that organizations login to the system anytime there is a change in your organization's information - i.e. officer contact information, membership changes, websites launched, meetings confirmed, etc.

This online process does NOT fully re-register your organization with UNCW. There are additional requirements that must be fulfilled by the 2009 re-egistration deadline which is Monday, Monday, September 28. Intent to Register Form - Use when petitioning to form a new student organization

Fundraising Permission Form - Use when soliciting businesses off campus. Information provided will be sent to UNCW's Advancement Office for approval. If needed, feel free to attached a more detailed list of businesses that your organization intends on soliciting.

Gift in Kind Guidelines - Follow these guidelines when you are considering to accept a non-cash gift on behalf of your organization and as you process the gift with the University.

Gift in Kind Form - Use this form when an individual or business wants to make a donation to your student organization in a form that is not cash (i.e. equipment, supplies, etc.). By completing this form and returning it to the Campus Activities & Involvement Center to be processed the donor can recieve a tax credit for the contribution.

Raffle Guidelines and Exemption Request Template - Use this template when requesting exemption status in order to conduct a raffle (i.e. 50/50, door prize giveaway, etc.). Also, be sure to refer to these guidelines as you plan any raffle that you may hold.

Road Race Checklist - Use this checklist when planning any type of road race on the UNCW campus.

Technology Request Form (Online Submission) - Use when requesting a new UNCW email account, webspace for your student organization, a Timmy folder to help your organization share files and maintain organization archives, a SeaPort Group or a SharePoint site for easy communication and file sharing. Also, be sure to complete this form again anytime you need to update who needs access to these resources.

Vendor Sponsorship Agreement Form - Use when bringing a vendor on campus as part of a fundraiser, or modify the form to fit with an event you hold off-campus to ensure that your organization is confirming a quality fundraiser.
