Documentation should validate the need for services on the individual's current level of functioning in the educational setting.
Documentation should be current within three years, but exceptions may be necessary dependent upon individual circumstances and disability. Confidentiality of records is required by law and maintained within our office. Disability Services can make known the specific nature of the disability only upon the student's written release.
In addition to academic accommodations, other disabilities may require special attention to access and/or safety issues. Therefore, documentation should address these accommodations.

Visual, Hearing and Physical Impairments
In order to receive services, all students with sensory, physical, or other health impairments are required to provide written documentation in the form of medical reports or a letter from a physician detailing the disability. This documentation should indicate current level of functioning and accommodations needed.
Learning Disabilities
Students with learning disabilities need to provide current (i.e.,
within the past three years ) documentation to validate the need for
services based on the individual's current level of functioning.
This substantiation of a learning disability should be conducted by
a qualified professional. A school plan such as an individualized education
program (IEP) or a 504 plan is insufficient documentation, but it can
be included as part of a more comprehensive assessment battery.
A comprehensive assessment battery and the resulting diagnostic report
should include a diagnostic interview, assessment of aptitude, academic
achievement, information processing and a diagnosis.
Evidence of a substantial limitation to learning or other major life
activity must be provided.
Report should include academic accommodations recommended. This documentation is reviewed and validated by the Director of Disability Services.

Speech Impairments
In order to receive services, students with a speech impairments must include evidence of a substantial limitation to learning or other major life activity, and should validate the need for services based on the individual's current level of functioning in the educational setting. Report should include accommodations recommended.
Other Impairments
Students with other conditions that may require accommodations and services should contact Disability Services to inquire as to the appropriate documentation necessary.
