Immunization Information
You can find immunization forms at Student Health Center site at www.uncw.edu/healthservices. Click on the immunization icon on the right side of the page.
Please be sure to complete all four pages of the form. You are welcome to attach immunization records from your doctor or school to the form and send them in all together.
