Foundations Portfolio Privacy Information

Students are encouraged to not place any sensitive personal information in their online portfolios. Some examples of sensitive personal information would include some of the following:

1. Social Security Number
2. Home Phone Number
3. Home Address
4. Date of Birth
5. Grades/Transcripts

Public School Student Information

Federal law prohibits posting of any identifiable information about students you may have worked with.

Some examples of things that should not be posted include, but is not limited to:

• Student's actual name or a made up name that appears real
• Grade level
• Age and birth date
• Teacher's name
• Test Scores/Grades
• Special accommodations

NOTE: Information about P-12 students and their teachers, their grade levels, test scores, special accommodations, and other information that might be considered confidential can be included in a candidate's web site only if:

• Students and/or teachers are identified by a number such as:
• Student A
• Student B
• Teacher 1
• Teacher 2

Photos of P-12 students may be used only IF a copy of the photograph and signed parental consent letter(s) has been placed on file in your department office for each student in the picture or pictures. No personally identifiable information can be included with the photo.
