INSURANCE

Summary of Coverage
May be used as proof of insurance if requested.

Participation Agreement for Film Studies Activities
To be signed by all students enrolled in production courses, internships, and any approved DIS or Honors project(s) in which student productions are involved.

Audio/Visual Image Talent Release and Hold Harmless Release of Claims
To be signed by anyone participating in the production but who is not in the class, for which the production has been assigned.

*Please Note: Talent Release Forms must be turned in, en masse, to the Film Studies Department office BEFORE any shoot commences.
