You will need to complete and submit both of the below forms:

1. Vendor Registration Form

• Download the form:
       http://www.uncw.edu/hub/vendor_registration_form.pdf
• Fill out the form, and return to UNCW Purchasing Services.

2. Contractor's Information Form

• Download the form:
        http://www.uncw.edu/hub/contractor_info.pdf
• Fill out the form, and return to the UNCW HUB Coordinator.
