Course Enrollment

Permission to Enroll, Directed Individual Study for Undergraduates

Directed Individual Study for Graduate Students

Must be signed by the Instructor, Dept. Chair, and Dean.
Permission to Enroll, Experiential Education Course Must be signed by the Faculty Supervisor, Dept. Chair, Agency Supervisor, and Dean.
Pass/Fail Option Must be signed by the Instructor and Dean.
Request to Allow Class Time Conflict Must be signed by the student, both course Instructors, and Dean.
Permission to enroll in Departmental Honors forms should be requested from the Honors Scholars Program Office.

 

Incomplete Grades

Assigning an Incomplete ("I") Grade Must be signed by the Instructor if form is printed. This form may also be submitted via e-mail. *
Report of Conversion of Incomplete Must be signed by the Instructor if form is printed. This form may also be submitted via e-mail. *

Request for Waiver/Substitution of Degree Requirements Must be signed by the Faculty Advisor, Dept. Chair, and Dean.
Transient Study Must be signed by the Student and Department Chair. (Residency requirement waivers must also be signed by the Dean.)
